ClinER-Electronic Medical (Patient) Record (EMR, EPR) (service to employees)

Doctors want to have the exact details of your past medical history and in many cases if there is no recorded data, the entire process has to start from scratch. EMR or EPRs is a new service that ensures our subscribers always receive the most appropriate diagnosis and treatment. In the UK, medical records are stored with your local GP and are updated when you visit the practice for advice or treatment. Certainly, you are the first to know if anything is wrong with your health and discuss potential cures. However, in many circumstances your medical history, that includes medical conditions you have suffered at the past, ongoing treatment, diseases that have affected your family and a lot more, may become urgently required for review by a specialist. This can happen either because you cannot communicate due to an accident, or simply because you cannot describe your medical history in proper and correct terms to a physician while on holiday.

Whatever the reason is, ClinER is the right solution while you are on holiday, in the office or walking down the street. It is your personal medical history written in the most accurate format, which belongs to you and is always with you. A smart software is used to decode all information related to your health status (past or current conditions, treatments, allergies, contact details of your relatives etc.) in the format of a mini CD or dongle as well as a mini-booklet. It is practically a “medical GPS” that is important to have in case of a medical emergency or also as a permanent and updatable record of your health history.

You may ask whether it is safe to have your medical history record formatted by an external provider and if there are any risks like passing personal information to the authorities (health and other) or even to life and health insurance providers. CliniServ has drawn particular attention towards these sensitive issues and has in place a procedure that ensures full compliance with the existing recommendations of the Royal College of General Practitioners on EMRs. Moreover, we have implemented a secured anonymity process in the generation, dispatch and management of the EMR information.

How it works: If you wish to have your EMR prepared, we post to you a ClinER scratch card together with a questionnaire. ClinER card has a 12 digit alphanumerical code that can be revealed only if scratched. You reveal your code, so you are the only one to know to whom these 12 digits correspond to. The next step is to phone us your code (we do not ask your personal details except for age and sex) and book an appointment to complete the EMR. This can be done in two ways: Either you let your GP to go through the questionnaire and then post it to us, or you may wish to complete the questionnaire yourself, provided you know all important details that need to be listed.

Your EMR will reach your address within 48hours.

We send you a hard copy of the EMR + a mini CD or dongle that can be read by any physician or hospital worldwide. In addition, a plastic case to accommodate your passport, the CD/dongle and the EMR hard copy, is also provided. If you are unable to handle it over to health authorities, it can easily be found in your belongings by hospital personnel or relatives, if an emergency arises. Payment is forwarded into our bank account quoting your ClinER card 12-digit reference number.

The ClinER card can also be used for anonymous blood testing either for BioCheck or BioCheck-Plus. ClinER has been broadcasted to all health authorities throughout the world (hospital, physicians), therefore you would expect local healthcare providers to be aware on the existence of your personal ClinER.
